CHNA Consent to Serve Agreement – 2025 (pdf) The final step in the nomination process. This requires a photograph and short bio and will be provided by the CHNA Nominations Committee Chair after receiving the “Nomination Acceptance” form. Deadline to complete is MAY 1, 2025.
